$150.00
fee due when submitted to the SCVA office

SCVA Facility Insurance Request Form
Usually
submitted with the Club Registration Form. No cost to registered
clubs.

This
document must be posted at all tryouts.

Individual Membership Form
$60.00
registration fee due when submitted to the SCVA office.
All
adults registering as a Club Director, Adminstrator, Team Representative,
Coach,
or Chaperone will need to pass a background check prior to the
registration

Junior
registration is normally submitted along with the team registration.
6. 2012/2013
Background Consent and Waiver Form
$20.00
processing fee is due when submitted to the SCVA office.
All
adults registering as a Club Director, Adminstrator, Team Representative,
Coach,
or Chaperone will need to pass a background check prior to the
registration

7. 2012/2013
Club Personnel Code of Ethics Form
All
administrators, directors, coaches, and other club personnel must
complete
the
Code of Ethics Form and it must be on file with the SCVA in order
for his/her
registration
to be considered complete.
8. 2012/2013
Chaperone Responsibility Form
All
chaperones must submit this form with the Adult Registration form
and
background screening form.
9. 2012/2013
Junior Girls Commitment Letter (Non San Diego areas)
All
junior players must complete this form and submitted to only one
club. This
form
may not be submitted prior to 4:00PM on October 18, 2012. A copy
of this
letter
must be submitted to the SCVA with the player's registration materials.
10. 2012/2013
Junior Girls Commitment Letter (San Diego areas)
All
junior players must complete this form and submitted to only one
club. This
form
may not be submitted prior to 4:00PM on October 18, 2012 for 14
and Under
players,
and 4:00PM on November 20, 2012 for 18 and Under and younger divisions.
A
copy of this letter must be submitted to the SCVA with the player's
registration
materials.
11. 2012/2013
Medical Release Form
This form is not submitted to the SCVA. All coaches should
have a copy of each
team member's medical release form.
12. 2012/2013
Team Registration Form
$15.00
registration fee is due when submitted to the SCVA office.
13. 2012/2013
Application for Change of Club - Junior Boys
This document must be completed and submitted to the SCVA by
4:00PM on
May 24, 2013.

2012/2013 Application for Change of Club - Junior Girls
This document must be completed and submitted to the SCVA during
the transfer
period. The transfer period is between December 16, 2012
and February 22, 2013.
No transfers will be allowed after 4:00PM on February 22, 2013..
