Official's Registration Forms for the 2012/2013 Season:

1. Background Screening Consent and Waiver Form

   All officials will need to have passed the USA Volleyball Background Screening test

   prior to any registration materials being accepted.  There will be no charge

   for this service for those officials working for SCVA events.  Officials need to be

   background screened every other year. For more information regarding the screening

2. Adult Registration Form

   A $60.00 registration fee is required when the registration form is submitted

   to the SCVA office.  All officials will need to have completed and passed the

   USA Volleyball Background Screening Process, prior to registering.  For more

3. Independent Contractor's Agreement

    Usually submitted with registration information.  No payments for services

    can be issued without this form on file in the SCVA office.

4. Officials' Availability Form

     Needs to be submitted with all registration documents.  Registration cannot be

     processed without it.
